Master Data Templates

The following templates are for FMMI agencies’ use in submitting master data maintenance requests to COD’s centralized processing area. All FMMI master data maintenance requests must be submitted via creation of a Remedy ticket to the COD Customer Resolution Center Online Help Desk, and the ticket must include a copy of the associated template.
